Medina Family Medical Clinic offers comprehensive care for patients of all ages. Our healthcare providers and staff are able to provide a variety of health services allowing patients to receive all of their health care needs within our office. We welcome new patients and can often accommodate same day appointments.

Make Appointments

On your first visit, we will begin a permanent record. Please bring any medications that you are taking, so we may accurately determine the dosage and frequency. We will ask you to complete a medical history and registration form. Please bring your insurance card and try to arrive at least fifteen minutes early for your first visit. If you have been under the care of another doctor, we may ask your permission to transfer some of your records. You may download our New Patient Forms and bring the completed forms with you.

Cancel Appointments

If you are unable to keep your appointment or you are going to be late, please call the office as soon as possible. This courtesy allows us to be of service to other patients.

Emergency Care

If you have an urgent problem, you may call the office to schedule an appointment. However, if you feel it is a life threatening emergency, call 911. Do not delay by calling our office first.

Prescription Refills

Our preferred method for refilling prescriptions is to have your pharmacist notify our office. Please contact them first and they will send us all the necessary information regarding your prescription. Since we have limited access to your medical records at night or on weekends, we are unable to refill prescriptions during those times.


At Medina Family Medicine Clinic, we have a strict patient confidentiality policy. Your medical record is strictly private and will not be shared with your employer, friends, or relatives without your written permission.


We are committed to providing you with the best possible care. If you have medical insurance, we are anxious to help you receive your maximum allowable benefits. In order to achieve these goals, we need your assistance and your understanding of our payment policies.

Payment for office visits are due at the time services are rendered (before being seen by the provider), unless payment arrangements have been approved in advance by our office. Co-payments and deductibles are required when you sign in at the front desk prior to being seen by the provider. We accept cash, check, and most major credit cards.

Returned checks will be subject to a $30.00 fee. These checks are collected upon by a third party vendor and cannot be paid at Medina Family Medical Clinic. If you do have a returned check for our facility, we will no longer be able to accept any future checks from you. Balances over 30 days old may be subject to additional collection fees and interest charges.

  • Every insurance company is different. Not all services are a covered benefit in all contracts. Not every insurance company will pay the same amount for any given service.
  • Your insurance is a contract between you and/or your employer and the insurance company. We are not a party to that contract. You are responsible for whatever your insurance company does not pay in a timely fashion.

We must emphasize that as health care providers, our relationship is with you, not your insurance company. It is not our responsibility to make your insurance company pay the full amount of the benefits to which you are entitled. While the filing of insurance claims is a courtesy that we extend to our patients, the charges are your responsibility from the date services are rendered. We realize that temporary financial problems may affect timely payment of your account. If such problems arise, we encourage you to contact us promptly for assistance in the management of your account.
